At first glance, the strange skin condition looks like an extreme case of blackheads—but the reality could be far more complicated.

The image appears to show a large, swollen area across the upper back and shoulder covered with numerous dark, crater-like openings. Because of its unusual appearance, however, it is impossible to determine the exact condition from a photograph alone. Images shared online can also be edited, staged, or artificially generated.

One possible explanation could be severe comedonal or nodulocystic acne, where blocked hair follicles become enlarged and inflamed. Other possibilities include clusters of open comedones, enlarged pores, or less common follicular disorders. Conditions such as chloracne can also cause unusual follicular lesions.

Whatever the cause, trying to squeeze, puncture, or remove the openings at home could make the situation much worse, potentially causing infection, bleeding, and permanent scars.
